This role will work on a high profile contract covering North and East London and co-funded by Big Lottery and the European Social Fund, and is a job that makes a real difference to people’s lives.
You will facilitate individuals’ access to the right opportunities at the right time. Using your knowledge and understanding of the surrounding community and its services, your role will focus on using your experience to connect people with the support they need in their own communities.
Working daily with clients, health professionals, volunteers and service providers, you will ensure that a person’s journey to better wellbeing is as easy as possible. You will use existing services and help to develop new local provision, liaise with primary and secondary health care professionals, checking in on progress and making sure we can evidence the outcomes of the service.
With an NVQ/SVQ Level 2 or above, or equivalent, you must have experience of working with external stakeholders, including primary and secondary care services. A track record of working with clients with complex and/or multiple barriers to achieve outcomes will be essential, as will an understanding of inclusion and mental health issues. You will be highly organised and be confident in using your own initiative to problem solve.
