Young People’s Transition Practitioner – Psychology Graduate
HF Mencap are looking to recruit a dynamic and experienced Psychology graduate with a 2:1 or 1st Class honours degree to work as a Young People’s Transition Practitioner
Now in its third year this innovative transition project sits with the third sector with involvement from NHS and Social Care services. It is led and managed by HF Mencap with support from the CCG, CAMHS and adult and children’s social care services.
Purpose of the Job
To work with Hammersmith and Fulham Mencap to deliver a transitions service to young people aged 14 – 25 and their families, with a learning disability or on the Autistic Spectrum Disorder (ASD), in transition, who also have a mental health difficulty to:
1. Develop a ‘needs led’ integrated pathway system for all young people with learning disabilities or autism requiring mental health support
2. Develop a ‘tapered approach to transition’ (1) to achieve a more integrated approach to transitions across children’s and adults services.

1 year fixed term contract based in the London Borough of Hammersmith and Fulham, salary £25,000 per year, 25 days annual leave plus statutory days.
Apply if you have a good degree in Psychology and Experience of working in a service delivery role with young people aged 14-17 and/or 18-25
Understanding of issues faced by people with learning disabilities and/or Autistic Spectrum Disorder (ASD)
