Young People’s Transition Practitioner – Psychology Graduate
HF Mencap are looking to recruit a dynamic and experienced Psychology graduate with a 2:1 or 1st Class honours degree to work as a Young People’s Transition Practitioner
Commissioned by Hammersmith and Fulham CCG, this innovative transition project sits with the third sector with involvement from NHS and Social Care services. It is led and managed by HF Mencap with support from the CCG, CAMHS and adult and children’s social care services.
The role involves supporting young people aged 14-25 years old with a learning disability and / or Autistic Spectrum Disorder, who also have a mental health disorder, as they make the transition from child to adult services. The role will involve working directly with the young person and their family to develop person-centered transition plans and providing transition support. Such support may include supporting the person with uncertainty surrounding transition and appropriate pathways, complexity around multiple service involvement, complexity around additional medical needs and/or the need for additional signposting to other services. The practitioner works with them and their families to develop an integrated pathway, signposting where they can get the skills or support to get them where they want and need to be.
The practitioner needs to be able to engage and work with professionals in other services as well as CAMHS, who will provide clinical consultation. He or she will need to be resilient, articulate, confident, and firm. Psychological competency is important in discussions and this is probably not a job that would suit someone who has gone straight from school and has just graduated: it needs experience of this sort of service delivery, this cohort of young people and a degree of maturity and life experience. 1-year fixed term contract based in the London Borough of Hammersmith and Fulham, salary £26,000 per year, 25 days annual leave plus statutory days.

Understanding of issues faced by people with learning disabilities and/or Autistic Spectrum Disorder (ASD)
Understanding of issues faced by people with mental health problems
